The Doctor’s Choice program is designed to allow SCL providers to select the patients you want to treat through our program and then “fast track” their applications so that those patients can begin treatment immediately.
The Doctor’s Choice program also allows SCL providers to treat candidates that may not have qualified under the standard SCL guidelines (age, finances, severity, etc.), but that you want to treat.
Types of individuals that our providers have referred to SCL through this program include:
- Military families
- Special needs patients
- First responders’ families
- Teachers’ families
- Current patients that may have fallen on tough times
- Kids that have “aged out” but are on their own or students
- Referring dentists “favor” patients
- Potential AR problem patients
- Employees’ families / friends
Steps To Follow If You Have A Fast Track Candidate
- Contact SCL and let us know that you have a child in mind.
- Let us know info on the situation and name, contact info of the candidate.
- Inform the family to complete an application at SCL Application
- Complete the form below. We will match this information with the application as soon as it comes in.
Traditional Request
Applicant submits completed application to SCL.
Qualified applicants are placed on waiting list to be sent to a provider.
Qualified applicant is sent to a provider for screening and decision on treatment.
Screening form is completed with a decision to treat and then returned to SCL.
SCL approves child for treatment and asks family to pay required financial investment within 30 days.
SCL informs Provider that child is officially assigned to start treatment
BANDING DAY!!!
Fast Track Request
Provider recommends applicant for program and informs SCL of name and phone number for Fast Track.
Applicant submits completed application to SCL.
SCL approves child for treatment and asks family to pay required financial investment within 30 days.
SCL informs Provider that child is officially assigned to start treatment.
